Chemotherapy is not necessary when complete evacuation of the gestational choriocarcinoma leads to a normalization of hCG levels
Strengths
- Gestational choriocarcinoma can be diagnosed after complete excision of the tumor when the level of human chorionic gonadotropin has already normalized.
- This international study of 80 patients whose human chorionic gonadotropin levels normalized after complete surgical evacuation of choriocarcinoma confirms that surveillance without chemotherapy is safe in the context of a highly selected patient population.
- No recurrence was observed during the median follow-up of 4 years.
This international multicenter study reports that selected patients with gestational choriocarcinoma, managed at referral centers for gestational trophoblastic disease, did not experience relapse when initial tumor evacuation was followed by normalization of human chorionic gonadotropin levels without any further treatment. Waiting for treatment may be a safe approach for these highly selected patients.
