Declare your patient
To declare a patient to the RMTQ, you must complete, have the patient sign (Quebec and outside Quebec) and send the documents below to the RMTQ nurse coordinator (Catherine DE RAVINEL):
- PATHOLOGY REPORT and any consultation are essential to the care
- All follow-up βhCG results and other documents if not available at the DSQ
- PATIENT EMAIL ADDRESS necessary for communications.
